It is specifically intended for use as a rapid screening instrument to identify individuals who may require a more comprehensive diagnostic evaluation for Autism Spectrum Disorder (ASD).
While the AQ is not a standalone diagnostic tool, scores above the established cutoff (29+) strongly suggest a profile consistent with clinical autism traits, particularly in populations previously classified under Asperger syndrome or high-functioning autism (HFA). It is valuable for research, clinical triage, and understanding the profile of strengths and weaknesses related to autism.
Construct
The AQ is designed to measure the underlying psychological construct of autistic traits distributed across the general population. It operationalizes this construct through five distinct domains, each comprising 10 items, reflecting core areas of difference observed in individuals with autism.
These five symptom clusters, which contribute to the overall profile of neurodivergence, include:
- Social Skill: Difficulties in reciprocal social interaction and understanding social cues.
- Attention Switching: Challenges in shifting focus between tasks or topics, often related to cognitive flexibility.
- Attention to Detail: A heightened focus on minute details, often at the expense of the “big picture.”

Validity
The initial validation study conducted by Baron-Cohen et al. (2001) demonstrated strong criterion validity. The scale successfully discriminated between an Autism sample (N=58), consisting of adults with Asperger syndrome or HFA, and a neurotypical control sample (N=174) drawn from the community.
Although the original study suggested a cutoff score of 32+ for optimal discrimination, subsequent validation efforts, such as those by Broadbent and colleagues (2013) in an Australian sample, refined this threshold. The updated analysis determined that a cutoff score of 29+ provided the best balance for screening purposes, yielding a false positive rate of less than 1% while maintaining reasonable sensitivity. It is important to note that using the cutoff of 29 fails to identify approximately 14.4% of people who meet the diagnostic criteria for Autism Spectrum Disorder (DSM-5 defined).
Reliability
The initial psychometric analysis of the AQ indicated adequate reliability. Specifically, the scale demonstrated suitable inter-rater reliability and acceptable test-retest reliability, suggesting the measure yields consistent results over time and across different administrators.
Internal consistency, which measures how well the items within the scale correlate with one another, was found to be moderate to high in the original study. This supports the notion that the 50 items collectively measure the cohesive construct of autistic traits. The robust performance across various samples confirms the AQ’s utility as a reliable screening instrument in adult populations.
Factor Analysis
The structure of the AQ is inherently based on a five-factor model corresponding to the five symptom clusters identified by the developers: Social Skill, Attention Switching, Attention to Detail, Communication, and Imagination. The items loading onto these factors are summed to provide subscale scores, allowing for a detailed profile of the respondent’s autistic traits across these distinct domains.
