Abstract
The 2-Minute Walk Test (2MWT) is an objective, standardized performance measure utilized to assess functional mobility, walking speed, and physical endurance in clinical populations. It requires the patient to walk the maximum distance possible on a flat, measured course within a two-minute period. This test provides valuable data on the patient’s capacity for sustained physical activity, offering insights into their overall fitness and functional status related to the **cardiopulmonary** and **locomotor systems**. Patients are permitted to use necessary walking aids or orthoses during the assessment.

Purpose
The primary purpose of the 2-Minute Walk Test is to provide a reliable, efficient, and objective assessment of an individual’s functional exercise capacity. It serves as a crucial tool for clinicians, particularly in fields such as physical therapy and pulmonary rehabilitation, to quantify the impact of disease or injury on mobility.
This measure is frequently used to establish baseline functional status, monitor disease progression over time, and evaluate the efficacy of therapeutic interventions, especially those targeting the **circulatory and respiratory system** or the **musculoskeletal system**. By focusing on the distance covered, it provides a direct measure of sustained locomotor effort.
Construct
The 2MWT measures the physical construct of **functional walking capacity** and **locomotor endurance**. It is considered a submaximal test that requires the patient to utilize maximum effort over a short duration, reflecting the physiological efficiency of oxygen transport and utilization necessary for sustained ambulation.
Specifically, the test captures multiple components of functional health, including the quality of the gait pattern, the capacity of the lower extremity musculature, and the functional reserve of the heart and lungs. The resulting distance score is interpreted as an objective indicator of the patient’s ability to perform necessary activities of daily living that require sustained walking.
Validity
The 2MWT demonstrates strong **criterion validity**, often showing high correlation with the established 6-Minute Walk Test (6MWT), particularly in populations where fatigue or time constraints make longer tests impractical. It is regarded as a valid performance measure for quantifying functional exercise tolerance across diverse clinical groups.
Furthermore, the test exhibits good **construct validity**, showing expected differences in performance between healthy individuals and patient populations suffering from conditions such as chronic obstructive pulmonary disease (COPD), heart failure, or various neurological and musculoskeletal disorders.
Reliability
The 2MWT is known for its high **test-retest reliability**, provided that strict standardization protocols are maintained, including consistent instruction, verbal encouragement, and environmental conditions. The objective nature of measuring total distance walked minimizes subjectivity, contributing to high **inter-rater reliability**.
Studies consistently show that the 2MWT provides stable results, making it an excellent outcome measure for assessing short-term changes in a patient’s functional status following acute rehabilitation or medical treatment.
Factor Analysis
As a single-score, timed physical assessment, the 2MWT is generally treated as a unidimensional performance measure of functional capacity. Therefore, formal factor analysis is not typically applied to decompose it into multiple latent factors, as is common with complex psychological questionnaires.
The variance in the total distance walked is understood to primarily reflect the integrated function of the patient’s locomotor, cardiovascular, and respiratory systems, rather than separate psychological or physical factors.
Instrument
Test Type: Performance (Objective Physical Assessment)
Format: Timed, observed physical assessment requiring maximum sustained effort.
Language Available: Instructions are required; commonly available in English and Dutch (Twee minuten looptest).
Population Group: Adults and Elderly (Geriatrics).
Age Group: Typically 18 years and older.
Population Details: Used for individuals with compromised function due to disorders of the **Bewegingsapparaat** (Musculoskeletal system), **Circulatie en ademhalingsstelsel** (Circulatory and respiratory system), and **Zenuwstelsel en zintuigen** (Nervous system and sensory organs).
Test Methodology: The patient walks continuously for two minutes along a flat, measured course, typically a long hallway or track, at a self-selected maximum effort pace. The patient is instructed to walk as far as possible without running. The use of walking aids or orthoses is allowed and must be documented. The total distance covered is recorded in meters.

Permissions & Fee and Test Year
The 2-Minute Walk Test is a non-proprietary performance measure first described in 1982. It is generally free for use in both clinical practice and research settings, requiring no specific licensing fee. The only required equipment includes a stopwatch, measuring tape, cones for marking the course, and a standardized walking track.

Items of the 2-Minute Walk Test / Twee minuten looptest (wandeltest)
IMPORTANT: The following scale items must be preserved in their original language and must not be changed in any way.
