Abstract
The Canadian Occupational Performance Measure (COPM) is a standardized, client-centered outcome measure designed primarily for use in occupational therapy. Its core purpose is to identify specific problems in daily functioning as perceived by the client and to establish relevant, measurable treatment goals. Purpose

Validity
The COPM is widely recognized for its strong face and content validity, particularly because the items assessed are directly nominated by the client, ensuring relevance to their daily life. Studies have consistently supported the construct validity, demonstrating that the COPM scores correlate logically with other measures of function, quality of life, and disability, though typically showing a stronger correlation with subjective measures than objective ones.
The measure exhibits responsiveness, indicating its ability to detect clinically significant changes in performance and satisfaction over time, which is essential for an outcome measure. Specific validation studies across diverse populations, including those with stroke, chronic pain, and pediatric conditions, confirm its applicability and meaningfulness across various clinical settings.
Reliability
The COPM demonstrates strong inter-rater reliability and test-retest reliability, particularly when administered by trained occupational therapy professionals. High internal consistency is generally not applicable in the traditional sense, as the scale items are chosen by the client and are heterogeneous; however, the scoring mechanism for performance and satisfaction demonstrates consistency.
Reliability studies generally support the stability of the performance and satisfaction scores when no clinical change has occurred. The structured interview format helps minimize variability in administration, contributing to the overall reliability of the measure.
Factor Analysis
Traditional factor analysis is not typically applied to the COPM in the same manner as multi-item questionnaires, primarily because the items are client-generated and specific to the individual, resulting in a unique item set for each administration. However, the overarching structure of the COPM is based on the theoretical factors of Self-care, Productivity, and Leisure, which are derived from the foundational models of occupational science.
Research focusing on the scoring mechanism often confirms that the two main scores—Performance and Satisfaction—function as distinct but related constructs, reflecting the dual nature of the client’s subjective experience of occupational engagement.

Test Methodology: The client identifies up to five problems in their occupational performance. For each identified problem, the client rates their current performance and their satisfaction with that performance on a 10-point scale. The scores are averaged to provide overall COPM Performance and Satisfaction scores.
