De DES is een 15-items vragenlijst waarmee de patiënt een medische beslissing evalueert. De 15 items van de vragenlijst zijn ingedeeld in drie Rasch-schalen: Tevredenheid-Onzekerheid, Geïnformeerde keuze en Beslissingscontrole.
